1. Deposit your ID with the staff to get a card number.
2. Submit this form.
3. Wait for staff confirmation before taking your pass.
Supported IDs: Resident Certificate (ARC), ISIC, University Student ID, Driver's License, or Passport.
We need this to reach you in case of emergencies while you are in the library.
